HOW DO I APPLY?
You, as a woman in need of financial assistance, need to contact your nearest certified mastectomy fitter. This may be listed in your area phone book under: breast prostheses, breast forms, post mastectomy, or mastectomy. You can download the application below and take it with you when you visit the place nearest you that offers post-mastectomy services. Ask the Certified Fitter there for assistance in filling out the application. Or, inform your fitter about the services available through The Breast Form Fund. She can then download or request an application. After she has sent in your application, The Breast Form Fund will contact your fitter about your eligibility to receive assistance. The fitter will fit you with your necessary products, send us an invoice, and we will pay the fitter’s company. Upon receipt of our check, she will give you your breast form and/or bras.

SHOW US YOUR BRA FUNDRAISER
The BFF has a major fundraising event, currently on a bi-ennial basis. (see Show Us Your Bra! ™) We produce a calendar every year, featuring entries from the contesst. All monies raised from the contest, gala, auction and calendar sales goes to the BFF. Also, Gazebo, the lingerie store in downtown Northampton, Massachusetts that founded the Fund, donates a percentage of all bra and prosthesis sales to the Fund.
